Thought i would say the medals;
Facerape: When you come to the point when the big guy's head sticks out of the door, use the chainsaw to split his head.
A winner is you: Beat the game xD
Pacifist: when you defeat the clown, don kill him, let him crawl away.
True face of evil: When you have defeated the clown hit him.
Madness mode: Beat madness mode.
